En este artà ­culo se distingue, para hacerlo ms claro, entre los trmites que se realizan cuando el cà ³nyuge extranjero est ya en los Estados Unidos y se sigue lo que se conoce como un ajuste de estatus. Y tambià ©n cuando se da la circunstancia de que est en otro paà ­s y se sigue lo que se llama procedimiento consular. Recordar, antes de empezar, que en los Estados Unidos producen los efectos migratorios los matrimonios entre una mujer y un varà ³n que los de personas del mismo sexo. Lo à ºnico que se pide en este à ºltimo caso, es que la boda se celebre en un lugar cuyas leyes permiten el matrimonio gay. Documentos para la tarjeta de residencia por matrimonio con ciudadano por ajuste de estatus Las personas extranjeras presentes en Estados Unidos pueden obtener la residencia, tambià ©n conocida como green card, por matrimonio con un ciudadano mediante un ajuste de estatus siempre y cuando hubieran ingresado al paà ­s de forma legal, con una visa o mediante el Programa de Exencià ³n de Visas. Es decir, no pueden hacerlo aquellas personas que ingresaron ilegalmente a los Estados Unidos. Los migrantes que se encuentren en esta situacià ³n deben asesorarse con un abogado de confianza antes de iniciar los trmites. Para los que pueden ajustar su estatus, pueden realizar en un solo paquete dos tramitaciones distintas: la peticià ³n de esposo y el ajuste de estatus. Los documentos son los siguientes: - Formulario I-130, peticià ³n de familiar - Copia certificada del acta de matrimonio. Si el casamiento se celebrà ³ en otro paà ­s, asegurarse de que cumple con todos los requisitos para ser considerado vlido en Estados Unidos. En este caso, si el documento o cualquier otro no est en inglà ©s, deber traducirse. El Servicio de Inmigracià ³n y Ciudadanà ­a (USCIS, por sus siglas en inglà ©s) se pondr en contacto para la cita de datos biomà ©tricos.   El siguiente paso ser recibir el permiso de trabajo, tambià ©n conocido como EAD, si se ha solicitado uno. Si la entrevista es exitosa, se sellar el pasaporte del cà ³nyuge extranjero con una Tarjeta de Residencia temporal, que sirve para viajar y para trabajar. En el plazo de uno o dos meses se recibir la green card de plstico por correo.   Finalmente, si la green card se recibià ³ antes de cumplir dos aà ±os de casado, ser de carcter condicional. Dicha condicionalidad la hay que levantar a los dos aà ±os. Peticià ³n de ciudadano para cà ³nyuge extranjero que est fuera de Estados Unidos En este caso el proceso es un poco distinto. Se inicia con la peticià ³n con el formulario I-130 y envindose el pago por la tramitacià ³n y la documentacià ³n de apoyo que sirve para probar que existe un matrimonio, la ciudadanà ­a del solicitante y la identidad del solicitado (ver ms arriba en apartado anterior). Una vez que la peticià ³n es aprobada, el USCIS la envà ­a al Centro Nacional de Visas (NVC, por sus siglas en inglà ©s). Y sern ellos los que contacten y comiencen a pedir documentos y planillas, seguir las instrucciones al pie de la letra. Pedirn, entre otras cosas, el affidavit of support y todo lo relativo a probar que se tienen ingresos suficientes para patrocinar. Tambià ©n se darn instrucciones para el examen mà ©dico y, finalmente, para la entrevista en el consulado o embajada. Llevar toda la documentacià ³n que se indique. Si se pasa la entrevista, se recibe una visa de inmigrante que se estampa en el pasaporte. A partir de ese momento hay 6 meses para ingresar a los Estados Unidos. Y en ese momento, al llegar a un puerto de entrada terrestre, marà ­timo o de aeropuerto, se sella de nuevo el pasaporte con un sello de ingreso que junto a la visa de inmigrante equivale a una green card temporal, hasta que se recibe la de plstico. Al igual que sucede con el caso anterior, la green card de plstico es condicional si se recibe antes de llevar dos aà ±os de casado. En es caso hay que levantar dicha condicionalidad. Finalmente, destacar que si una persona est fuera de los Estados Unidos y piden a su nombre una green card, es muy difà ­cil que a partir de ese momento aprueben una peticià ³n de visa de turista. A tener en cuenta antes de iniciar los trmites para pedir a un cà ³nyuge De todas las cosas, dos destacan por su importancia y posibles consecuencias. En primer lugar hay que asegurarse de que se tienen ingresos suficientes o que se cuenta con un co-patrocinador. El USCIS no aprueba peticiones si no se cumple este requisito, no hay excepciones.   Y, en segundo lugar, informarse sobre las causas de denegacià ³n de la residencia. Y es que no es suficiente estar casado con un ciudadano. Hay que ser elegible para ingresar a los Estados Unidos y convertirse en residente. En algunos casos no es posible arreglar el problema. En otros es cuestià ³n de esperar a que pase el tiempo de castigo o, cuando se puede, pedir un perdà ³n, tambià ©n conocido como waiver o permiso. Esto à ºltimo hay que tenerlo especialmente en cuenta cuando el problema viene del hecho de haber estado ilegalmente en los Estados Unidos. Esto à ºltimo es un problema cuando se superan los 6 meses de ilegalidad  y se est fuera de Estados Unidos sin importar cà ³mo se habà ­a ingresado o cuando se est dentro y se ingresà ³ ilegalmente. Tambià ©n hay que tener en cuenta que en cosas del amor, las situaciones pueden cambiar. Si hay un cambio de opinià ³n por parte del ciudadano, podrà ­a poner fin a la peticià ³n.  Tambià ©n tener en cuenta que el divorcio, por parte de cualquiera de los cà ³nyuges, puede tener efectos migratorios importantes. Stereotyping of Minority Students      Ã‚  Ã‚  Ã‚  Ã‚   Throughout most of my school years, before attending MCTC, I had never experienced inter-racial schooling.   I went to a small high school with about twenty minority students.   These students were not outcasts they were treated as any other non-minority student.   Therefore, I had little experience with minorities in a non-minority school.   After the speech that was given by Michael Jefferson, I was more informed on the effects of stereotyping of minority students in a non-minority school.      Ã‚  Ã‚  Ã‚  Ã‚   Stereotyping may cause students and teachers to view minority students on a non-minority campus as inferior.   This may cause some teachers or faculty members to believe that these minority students are not as capable of achieving the same quality of work as non-minority students. Scholarships are sometimes given out to non0minority students due to their racial background.   I suppose this happens due to the stereotype of minority students not being capable of the educational work as that of other students.   Classroom behavior toward minority students may be shown in appropriate, and/or unrecognized. For instance, a teacher may call on a non-minority student before a minority student.   The teacher may pick on the non-minority due to a belief that these students care about their schooling, whereas minority students are being pre-judged without the chance to really learn.   Some people even believe that combining the different minorities may cause problems.      Ã‚  Ã‚  Ã‚  Ã‚   Combining minority with non-minority students can create some conflict.   Some non-minority students, this attitude of aggression may be brought on by "close-minded" non-minority students.   These students let their attitude take over their learning time, by worrying about the other minority students.   Each student, whether thy be minority or non-minority, must put their mind on learning than on another minority on the campus. Benign Prostate Hyperplasia

The prostate gland is an exocrine gland found only in the male. Exocrine glands secrete chemicals to the outside of the body. It is a glandular organ and is found just below the bladder and at the top of the penis. It is normally about the size of a walnut and it surrounds the urethra which is the tube that brings urine from the bladder to the outside of the body. It is made of fibrous tissue and some muscle tissue. Anatomy The gland is divided into zones or lobes. The peripheral zone is the outside part of the prostate and this is where most prostate cancers are found.The central zone makes up about 25% of the gland and the transition zone makes up about 5% of the gland. The transition zone, however, is where are the hypertrophy happens. This part of the prostate is closest to the urethra so as it grows it puts pressure on and squeezes the urethra causing problems with urination. However, in benign prostate hyperplasia, the prostate grows and begins to cause the person problems. It is usually an age related disease which is nonmalignant. Large lesions grow on that central section of the prostate.It is believed that 60% of men over the age of 60 have BPH (Porth & Matfin, 2007). The exact cause is unknown but there are some risk factors which include age, history, race, dietary fat, and hormonal factors. The older one gets, the more likely to have it and African American men get it most often. When young the gland grows with the hormones testosterone and dihydrotestosterone and it is believed that in some men the hormones do not slow down and stop when they should so the gland continues to grow past what would be normal. Signs and SymptomsBPH causes compression on the urethra so it causes partial or complete obstruction of urine flow. As the obstruction increases (prostate gets larger), acute or emergent retention of the urine may occur with distention of the bladder. The urine that is left in the bladder when a man tries to urinate and is not able to empty his bladder causes a frequency to occur, consistently trying to empty the bladder. This usually becomes worse at night. When there is a lot of bladder distention accidents may happen every time pressure is put on the abdomen or when coughing causing pressure inside the abdomen.When urine is not completely emptied from the bladder the chances of a urinary infection occurs and this sometimes happens frequently. All of this develops in stages over time so there is often changes in the kidney and ureters because of the long time back up of urine causing hydronephosis which eventually begins to kill kidney gomeruli. If not treated early this can lead to renal failure. Diagnosis When making a diagnosis of BPH seven questions are at first ask. These questions are regarding incomplete emptying, frequency, intermittency, urgency, weak stream, straining, and nocturia.A final question relates to the quality of life the patient is having related to this problem. A history, physical exam and rectal exam is done. Also to be done are urinalysis, PSA, and serum creatinine as well as other blood tests. The urinalysis is done to detect bacteria or infection, the serum creatinine is used to estimate whether there is renal function problems and the PSA is used to screen for cancer. Our Patient JJ has BPH and he has symptoms of frequent or urgent need to urinate, he also has developed cysts in his urine and a urinary tract infection.All of these symptoms make sense because they are symptoms of the enlarged prostate putting pressure on the urethra. The urinary tract infection has occurred because of the retention of urine in the bladder. His treatment plan is medications and TURP or TIP. Treatment Treatment in the early stages is aimed at symptom relief, then there is a time of watchful waiting which is suggested by most physicians. There are presently medications that work pretty well for some time. These are Finasteride which reduces prostate size by blocking the effect of androgens on the prostate.This drug also cause atrophy of the prostate cells which can cause a 20% reduction in volume. The onset of the drug takes some time, sometimes 3-6 months but it usually works for some time. The side effects may be erectile dysfunction and decreased libido. There can also be the surgical removal of the enlarged prostate gland. This surgery can be done through the urethra, through a suprapubic incision, or through an incision in the peritoneum. The most commonly used way is called a TURP which is transurethral prostatectomy or through the urethra.The prostate is a very vascular organ and though this procedure is fairly simple to do, there is a great deal of bleeding on average and irrigations are often necessary post-operatively. There are presently many experimental treatments that it is hoped will provide less side effects. These include laser surgery, transurethral vaporization, transurethral microwave therapy, and transurethral needle ablation. There is also a new technique using balloon dilation but these effects at this time seem to be transitory. Stents can also be placed in those men that have major heart of lung issues and cannot tolerate surgical procedures.Treatment Plan So, JJ had symptoms of BPH which was treated by medication but his symptoms persisted and he had a TURP. This procedure after recovery relieved his symptoms and he is progressing quite well with most of his symptomology gone at this time. Conclusion BPH which is thought of as a cancerous condition of the prostate is a benign condition of the prostate. The symptoms which it causes are usually the method of treatment in the beginning and this may last for some time, but there may be a later need for a more aggressive method of treatment which at this time is the TURP.There are side effects of this surgery, some of them immediate because of the vascular condition of the prostate. This surgery, at this time usually solves the problem long term. There are many more less inv asive procedures being studied at this time. Our patient JJ is doing quite well after a TURP and will most likely not need a further procedure.
